Canonically, Sans could have much more than one HP. As we see in the genocide route, you do over 999999 damage to him, while he is exhausted aswell. Since he is tired, his guard will drop, and as you do over 999999 damage to him, he dies. This is something that everybody puts aside because sans 'only has 1 hp'.
The only way to progress in his fight is to attack. If you use an item or "Check" him the battle will not progress and he will repeat the last move he just did. Whenever you get hit by one of his attacks, you take Karmic damage. Essentially poison damage in any other game.
It takes 21 turns of attacking him for him to die. Sparing or using an item does not progress the fight only attacking him will. You must press the FIGHT button fourteen times to beat him.
Legendary Hero is a food item sold at the MTT-Brand Burger Emporium. Consuming it heals 40 HP, raises AT by 4 for one battle, and makes a fanfare sound.

If the protagonist loses to him three times, Papyrus grows tired of capturing them and offers to allow them to skip his battle and progress. In the Genocide Route, Papyrus instantly offers to spare the protagonist. However, a single hit kills him. Sparing him aborts the Genocide Route.

A frisk in this context is supposed to be a limited search of the person's outer clothing—for weapons. If you're aiming for the Genocide route, then you'll have no choice but to kill every creature you come across in the game. Undyne the Undying is one of them, and she won't go down without a fight.
