Dr. Goldberger gave a pretty decent answer. +/- means “maybe yes, maybe no”. It's another way to say “possibly”. So, the good doctor gave you an example: ARF d/t CHF exac +/- COPD.
It is merely called the “at sign” or “at symbol.” It originated as a shortcut for scribes writing the Latin word “ad” meaning at… usually used in lists of prices. Other languages have various names for the symbol, usually based on the shape of it.
