Although both the notification and the "activity" will be cancelled when you "unlike" the picture, the short answer is yes, the action might be visible to someone if they happen to access FB during the window of time when it was valid, or if they had their notifications delivered by e-mail.

FYI: Instagram's official page states: If you've accidentally liked a photo and then unliked it, the person who posted it will still get a push notification if they have notifications turned on. BUT very soon after you remove a like, the like alert will disappear from other people's Activity.
For example, to choose who can see the movies you've liked, scroll down to the movies section, click in the top-right corner and then select Edit Privacy. To choose who can see what Pages you like, scroll down to your Likes section, then click in the top-right and select Edit the Privacy of Your Likes.
Delete a 'Like'
You can't really "delete" a like so much as you can unlike something. The easiest way to do this is from the Activity Log -- simply find the item (post, photo, comment, or link) you want to unlike and click the pencil icon to the right of it. How to unlike pages on Facebook:
- Open Facebook, log in to your account, and navigate to your profile.
- Click on the "More" tab and select "Likes" from the drop-down menu.
- Find the Facebook page you want to unlike.
- Click the page's drop-down menu that says "Liked" and select "Unlike"
To remove someone who likes your Page:
- Click Settings at the top of your Page.
- Click People and Other Pages in the left column.
- Click to check the box next to the person you want to remove.
- Click and select Remove From Page Likes.
- Click Confirm.
Scroll down until you find a page you want to unlike, then tap on it, this will bring you to that page. You'll see blue thumbs up with "Liked" written underneath, tap that, and the option appears to unlike the page, tap it.
Go to the Page on Facebook. Find the button that says “Following”. Click on it and scroll down to “Unfollow this Page”. Alternatively, if you don't want to leave the Page but you no longer want to receive notifications, you can simply turn Notifications off by clicking “All Off” under the Notifications section.

Someone who likes a Page can choose to unfollow the Page. In that case the person is still shown as liking the Page, but they will not see all of the Page's posts. It's also possible for someone to choose to follow a Page but not like it.

When you click on Like your name is added to the list of Likes for the comment/post and the link is changed to Unlike--this is probably to stop multiple Likes from the same person.
To unlike a Page:
- Go to the Page by clicking its name in your News Feed or searching for it.
- Below the Page's cover photo, hover over Liked.
- Select Unlike this Page from the dropdown menu.
